At the point of referral a detailed case management assessment is completed to establish client need. This important assessment, identifies the cognitive and consequent behavioural deficits, on which to build the future rehabilitation programme for the client.

A rehabilitation plan will document and identify overall aims of rehabilitation, set objectives and provide targets by which the intervention can be measured.

Regular 12 weekly reviews monitor the progress of the case, and allow for evaluation of goals and targets to take place.
